Important Things to Know for 2009-2010


  1. Section 125 Cafeteria Plan: The Lubbock-Cooper ISD Cafeteria plan year is September 1, 2009 through August 31, 2010. Voluntary benefit plan changes for this enrollment will be effective 10/1/2009.


  2. Flexible Spending Accounts (FSA): MGM is the Third Party Administrator (TPA) for Lubbock-Cooper ISD for plan year September 1, 2009 through August 31, 2010. Eligible Expenses must be incurred in the plan year. Direct deposit is available. You may also use your new MGM Flex Visa Card for Medical Reimbursement. Please remember to keep your receipts if you use your Flex Visa Card.


  3. Dental: Effective 10/1/2009, Met Life is the dental provider for Lubbock Cooper ISD. Lubbock Cooper ISD will be pay for the Employee only premium. Dental effective date is 10/01/2009.


  4. Vision: Block Vision is the vision provider for Lubbock-Cooper ISD. Members can receive new lenses, frames and eye exams once a year. Great News!! Lubbock Cooper ISD will be paying for employee only vision coverage effective 10/01/2009.


  5. Unum Disability: Unum Provident is the disability provider for Lubbock Cooper ISD. All new increases in coverage are subject to pre-existing condition limitations.


  6. Voluntary Group Term Life: 5Star Life Insurance is the voluntary life insurance provider for Lubbock-Cooper ISD effective 10/1/2009. Current Employees can elect up to $150,000 life insurance, spouse can elect up to 50% of the employee election and children up to $10,000. Current employees applying for additional coverage must complete and Evidence of Insurability Form. New Employees to the district are guaranteed issued up to $150,000 with no medical questions asked. Spouses of new employees are guaranteed issued to $50,000 and children of new employees are guaranteed issued to $10,000.


  7. Cancer: Transamerica is the cancer provider for Lubbock-Cooper effective 10/1/2009. Employees will have two options to choose from. Low Option and high option cancer coverage will be available. Both plans include first occurrence benefit, radiation and chemotherapy, specified disease and ICU benefit.


  8. Plan offerings to Lubbock-Cooper ISD employees include: Met Life Dental, Block Vision, Unum Disability, Transamerica Cancer, 5Star Life Group Term Life, Texas Life Individual Life, and Flexible Spending Accounts.
